All J1s odacians pls meet Justin outside odac room at 7.15am on mon 23rd feb

Things to bring and submit:
-XBlaze consent form
-events form
-2 passport size photos(if dun hav pls go take tis weekend)
-5 photocopy of passport
-5 photocopy of NRIC

If your passport have expired or is going to be expired soon, pls go renew it asap. Once you get yr new passport go find Ms Tan to update her on yr new passport no. and the expiry date.

Even if you cannot hand in some of the items, pls still meet justin at 7.15am cuz he need to note down the reasons and report to Ms Tan. Pls cooperate with him and make his job easier k? Try not to be late so that all the forms can be in b4 morning assembly start.
